Obituary of Andrzej Kozak

KOZAK, Andrzej - Passed away peacefully at his residence surrounded by his family on January 25, 2009 at the age of 67. Beloved husband of Jadwiga. Loving father of Anna (Brad Clement) and Adam Kozak and caring step-father of Jacob (Monika), Pawel (Anna) and Marcin. He will be fondly remembered by his grandchildren Nicole, Matthew and Robert and sadly missed by his brother Grzegorz, as well as his many close friends and family. Family and friends will be received at the GEORGE DARTE FUNERAL CHAPEL, 585 Carlton St., on Tuesday January 27, 2009 from 6 - 9 p.m. Funeral Liturgy will be celebrated at Our Lady of Perpetual Help Church on Wednesday January 28, 2009 followed by interment at Victoria Lawn Cemetery. In lieu of flowers, donations made to the Juravinski Cancer Centre or the Canadian Cancer Society would be appreciated by the family.
